To Request a Copy of a Collision Report
If you were involved in a collision and would like to request a copy of the collision report, complete the "Request for Copy of Collision Report" form. You may complete the report online (See Instructions below), then print it and mail it along with the $5 fee to:

If you wish to file a report for a collision, please contact your local police department, county sheriff's office or Washington State Patrol detachment for a collision report form.

What information do you need from me in order to fill a request?
|

Please provide as much information as possible describing the date and location of the collision, as well as the names of the involved parties.
|

How far back in time are collision reports available?
|
|
Collision reports are available for the last seven years.
|
|
How long will it take to receive my copy of the collision report?
|
|
As soon as your request is received, a search begins for the collision report. Depending upon how soon the collision report is received in Olympia, it can take anywhere from a few days to up to 75 days to locate and provide you the requested collision report.

Instructions for filling out PDFs online
|
|
In areas where you supply text, click on the line where you would like to fill in the information. A cursor will appear. When the cursor is blinking, start typing.
When you are done filling in a field, click the tab button to move to the next field.
In an area where you would normally place check marks, place your cursor over the area and a pointer (hand with pointed finger) will appear. A single mouse click will place a check mark in the check box. If you click the check box again, the check mark will disappear.
