Submittal Information Packet
Due to the requirements outlined in 49
CFR Part 26, "Participation by Disadvantaged Business Enterprises
in Department of Transportation Financial Assistance Programs", the form,
"Submittal Information Packet", is required on all submittals for Request
for Proposals (RFP) and Request for Qualifications (RFQ). If this form
is not completed in its entirety, including information for all sub-consultants,
your submittal will not be considered for the project. You can obtain
this form by clicking on the forms name below, or you may obtain a hard
copy by contacting the e-mailing your request for the form with your name,
firm name, address, or fax number or calling (360) 705-7104 and providing
us with your name, firm name, address, or fax number. A hard copy will
be sent either by e-mail, fax, or regular mail service.
